Family Health and Wellbeing Service for Children
There is now a new Family Health and Wellbeing Service available for children between the ages of 0 to 19.
Parent's with pre school aged children can telephone the hub 03003033923 for health advice regarding their child Monday to Friday 9.00am until 5.00pm excluding bank holidays.
If you would like to refer a school aged child or young person to the service please get consent from the child's parent, complete the referral form attached with as much information as possible and specify what support is being requested. Then please send the form via e-mail to the e-mail address on the form.
Please Note Parents can self-refer their school aged child into the service by telephoning our 0 to 19 Hub at Kidsgrove on 03003033923 Monday to Friday 9.00am until 5.00pm excluding bank holidays.
Please also find attached our electronic business card advertising our ChatHealth service. The ChatHealth service enables parents to text into the service if they have any health and wellbeing concerns about their child. This service is available for all parents/carers with children aged 0 to 19 who live in Staffordshire. The service is available Monday to Friday 9.00am to 5.00pm excluding bank holidays.
The ChatHealth service also enables young people aged 11 to 19 years who have concerns about their health and wellbeing to text into our service please see attached the information to share with them. Parents/carers and young people may also find useful information realting to health and wellbeing by accessing the FHWB website.
